Montage Medical Group Medicare patients can enroll in Montage Health Connected Care, an at-home extension of the doctor’s office to help you manage your health in between office visits.

A clinical care team collects, analyzes, and responds to your health needs by supporting you with referrals, medications, lab work, coaching, and more.

What is Montage Health Connected Care?

Montage Health Connected Care provides 24/7 support for seniors to receive help even when the office is closed. Additionally, patients with high blood pressure, congestive heart failure, or type 2 diabetes receive easy-to-use devices to take vital readings from home. Our team receives a patient’s readings and automatically reviews them to make sure everything is normal. For diabetes patients, we also provide glucometers with unlimited strips and lancets.

Peace of mind for you and your loved ones

Montage Health Connected Care coordinates with your doctor to help manage your treatment in between office visits. Patients receive ongoing support when they are at home.

Montage Health Connected Care is covered through Medicare. Patients may have a monthly co-pay depending on the insurance plan.
