What is heart failure?
Heart failure refers to the heart’s inability to effectively pump blood through the body. We prefer to call it “heart dysfunction,” which is a better description of your condition.

Montage Medical Group patients who have received a heart failure diagnosis can benefit from the heart failure clinic.
What to expect
In our “heart dysfunction” clinic, you have an entire team supporting you. This team consists of your cardiologist, an advanced practice provider (such as a nurse practitioner or physician assistant), registered nurses, medical assistants, social workers, dietitians, and cardiac rehabilitation specialists.
Your initial appointment will be longer than your typical appointments. You will likely meet with an advanced practice provider who will spend more time with you to help you understand your diagnosis and individual treatment plan. Your cardiologist is always involved in your care plan. Our goal is to set you up for success in managing your heart health.
We will discuss:
- What is heart failure
- What medications you need to take and associated side effects
- Lifestyle changes to help you best manage your heart health
- Symptoms of heart failure and how to identify when your symptoms are getting worse
- Recent diagnostic testing and blood work pertaining to your heart
- When to call your healthcare provider
- How often you will need to return to the clinic for evaluation and treatment. In the beginning, it may be as often as every few weeks to help you understand your diagnosis, how to manage your medications and symptoms, and when to be concerned
