Referrals and insurance
Your insurance plan may require a referral or authorization from your insurance plan and/or primary care doctor. Please check with your insurance plan before your appointment to confirm what is required. For help getting a referral or questions, call us at (831) 648-7200.
Before making an appointment, ask your insurance provider if its network includes Montage Orthopedics and Sports Medicine. If your insurance does not fully cover your care and assigns a patient responsibility, you will receive a bill from us.

Copays
You might have a copay, depending on your insurance plan. You’ll need to pay it at your appointment.
Paying your bill
For your convenience, we encourage you to use our secure online payment options through MyChart, where you can quickly view and pay your balance. Payment is due within 30 days of receiving your bill.
You may also:
- Mail a check or money order
- Pay using Visa or MasterCard
Appointment cancellations
Call (831) 648-7200 at least 4 hours in advance if you need to cancel your appointment. By doing so, you give us time to schedule another patient in your place.
No-show policy
“No-shows” are cancellations with less than 4-hour notice. See our policy for:
- First no-show: You and your primary care doctor will get a letter about the missed visit.
- Second no-show in 12 months: You’ll receive a bill for $25.
- Third no-show in 12 months: We won’t schedule you again.
We understand emergencies happen. If you call to explain, we may excuse your absence.
