Montage Health (“Montage Health,” “we,” “us,”) provides websites (each a “site” and collectively the “sites,”) which allow users to view health-related information and access additional services. These websites include:

It is important for you to know how we handle the information we receive from you through the internet and our sites. You can visit our sites without revealing any personal information; however, you are required to provide personal information if you use the sites to register for a class, lecture, or event, subscribe to a newsletter, make an online donation, or otherwise communicate with us through your use of the site.
Nature of information we collect
We collect information that you provide to us, which may include your name, contact information, and other personally identifiable information. As described herein, we also collect system-related information, such as an IP address, browser type and operating system type.

We may use or disclose your information to run our business, including but not limited to: providing customer service, conducting research and analysis, sending marketing communications, providing cloud computing infrastructure, and/or donation or payment processing.

We may also disclose your information to subsidiaries, parent companies, and/or companies with which or by which we have, respectively, merged or been acquired.

Protecting your information is a top priority for Montage Health. In addition to applying internal confidentiality policies that govern access and use of information by clinicians and staff, we have implemented robust physical, administrative, and technical security features and methods designed to safeguard your data in our information systems. These features and methods include the use of encryption (such as Secure Socket Layer (SSL) system), firewalls, monitoring, access controls, and other controls where appropriate. Despite these measures, the confidentiality of any communication or material transmitted to or from us via the sites cannot be guaranteed. You agree and stipulate that use of the sites is at your own risk, and you will not hold us responsible for any breach of security that is not solely caused by our negligence.
Links to outside websites
We use web log information to make our sites more useful to visitors; to design a more positive user experience; to resolve user, hardware, and software problems; and for security purposes. All web logs are stored securely, and may only be accessed by Montage Health employees or designees on a need-to-know basis for a specific purpose. This sort of information is used only in aggregate, and to maintain your anonymity, we do not associate web log information, including IP addresses, with records containing personal information. Our sites use Google Analytics and other analytics platforms to better understand usage of our sites. Similar to most websites and best practices, we gather certain information automatically, and this information is stored for no longer than reasonably useful to carry out its legitimate business purpose, or as legally required. These logs may contain the internet domain from which you access the site (such as google.com, comcast.net, etc.), the IP address which is automatically assigned to your computer when you get on the internet, the type of browser and operating system you use, the date and time you visited, the pages or mobile screens you viewed, anonymous demographic data, and the address of the website you linked from, if any.
We, and our partners, may place internet “cookies” or similar technologies on the computer hard drives of visitors to the sites. Cookies help us obtain information about your use of our sites and consist of a unique identifier; they do not contain personally identifiable information about you or your health history.
Most web browsers automatically accept cookies; however you may have software on your computer that allows you to decline or deactivate cookies. If you reject or block cookies from our sites, some parts of the sites may not work properly for you. For instructions on how to remove cookies from your hard drive, go to your browser’s website for detailed instructions. Chrome, Firefox, Internet Explorer, Safari, and Opera are commonly used browsers. Registration forms and credit card transactions
On certain pages of the sites, we may give you the option to provide us with personal information such as your name, address, phone number, email address, demographic information, or preferences. For instance, you may register for a class, lecture, or event on one of our sites, or, submit a ‘Contact Us’ secure messaging web form to provide feedback. We only gather this personal information with your knowledge, permission, and participation. If you are not comfortable with this, you can choose not to use this part of the site.
If you provide us with your credit card number for class, event, or lecture registration, making a donation, or other payments, we will treat your credit card number in a secure manner by submitting the number directly to our credit card processing service providers through a frame on the sites.
Email communication to you
Montage Health uses third-party vendors to help us manage some of our email and communications with you. Your email address is never used for any purpose other than to communicate with you on our behalf. When you click on a link in an email, you may temporarily be redirected through one of the vendor’s servers (this process will be invisible to you) which will register that you’ve clicked on that link and have visited our sites. This information is for tracking purposes to measure email metrics such as opens, link clicks, delivery effectiveness, and additional metrics. Your ability to opt out of these email communications is discussed below.
Sending messages to Montage Health
Comments or questions sent to us using email or secure messaging web forms may be shared with a customer service representative, employee, medical personnel, or agent who is most able to address your inquiry. We make reasonable efforts to respond in a timely fashion when communications are received. Once we have responded to your communication, it is discarded or archived, depending on the nature of the inquiry.
The email functionality on our sites does not provide a completely secure and confidential means of communication. It’s possible that your email communication may be accessed or viewed by another internet user without your knowledge or permission while in transit to us. If you wish to keep your communications private, do not contact us via email.
We do not knowingly solicit or collect personal information from, nor do the sites provide content for, children under the age of 18. If we are made aware that we are in possession of information from a child under 18, we will delete it. If you believe that we are in possession of any such information, please contact us through the means described below.
Data integrity and opt-out
To update or request deletion of your personal information collected on our sites, or to remove yourself from a mailing list, please use the information found in the Contact Us section below.
If you receive a promotional email or newsletter from us, you can unsubscribe at any time by following the opt-out procedure within the footer of the email, as part of the CAN-SPAM Act. Please note, however, that you cannot opt out of certain messages, such as an email confirming your class, event, lecture registration, or donation receipt.
Retention of information
We may retain your information for as long as we deem reasonable or as long as required by law.
Users outside the United States
All of the information that you provide to us will be stored and processed in the United States. If you access our websites from outside of the United States, your use of the sites constitutes consent to the transfer of your information into the United States. The laws in the United States may not offer the same degree of privacy protection as the laws in your jurisdiction. We do not knowingly collect personal information about individuals residing in the European Union.
