Responding to Community Hospital’s Spring 2026 Leapfrog Hospital Safety Grade

At Montage Health, patient safety is our highest responsibility. When our performance does not fully meet the expectations we set for ourselves, we believe it is essential to acknowledge it, learn from it, and take meaningful action.

As of today, Community Hospital of the Monterey Peninsula holds a “C” Hospital Safety Grade from Leapfrog. This spring 2026 grade reflects hospital performance using CMS and Leapfrog survey data drawn from mid‑2022 through 2025. While our underlying safety performance score has improved since the previous reporting period, we still have more work to do and we take that responsibility very seriously.

We have already strengthened our safety and quality infrastructure by restructuring our Quality Management department to ensure the right clinical expertise is in place to meet patient safety needs. This includes an all-nurse staffing model as well as investing in our infection prevention staffing to help us further reduce healthcare‑associated infections. In addition, our Interdisciplinary Quality Committee has been broadened to bring together subject‑matter experts in key safety domains. We are also enhancing our Patient & Family Advisory Council by expanding in‑person participation and creating scalable ways to gather input from thousands of community members, ensuring the voices of patients and families help shape decisions about care. This work is ongoing, and we will continue to measure, monitor, and report our progress.

It is also important to acknowledge that Community Hospital is rigorously evaluated by multiple independent organizations. Earlier this year, Community Hospital was named among America’s 250 Best Hospitals™ by Healthgrades for the fourth consecutive year, placing it in the top five percent of hospitals nationwide for consistently delivering exceptional clinical outcomes.

We remain fully accountable to our patients and community, and we are committed to making measurable, sustained improvements in safety and quality to strengthen the trust placed in us every day.
